SCOPE OF POSITION:

The Bachelor’s level practitioner will assess client and family needs and evaluate, coordinate, and ensure necessary services are provided; conduct psychosocial evaluations, conduct functional assessments provide community based or in-home services to clients; assist individuals by using such activities as delineating alternatives, helping to articulate goals, and providing needed information, service, and treatment.

ESSENTIAL FUNCTIONS:

  • Conduct psychosocial evaluations via the collection of data about patients through interview, case history, and/or observational techniques; evaluate data to identify cause of problems and determine proper therapeutic approach or referral to other specialists.
  • Conduct functional assessments.
  • Coordinate discharge and transition of patients.
  • Coordinate emergency crisis services as necessary; make immediate clinical assessments and respond accordingly to accepted crisis intervention methods and techniques; coordinate other services as appropriate.
  • Act as a client advocate in order to coordinate required services
  • Prepare appropriate documentation to bill services by established scheduled due dates submitted by Florida Therapy Services, Inc.
  • Maintain and generate necessary clinical and administrative documentation in a timely manner.
  • Review client charts to ensure quality assurance and compliance with state and federal standards and regulations. Participate in agency training as required.
  • Attend all scheduled supervision meetings with Program Director
  • Participate in continuous FTS quality improvement activities.

MINIMUM QUALIFICATIONS:

  • A bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, nursing, rehabilitation, special education, health education, or a related human services field.
  • Training in the treatment of behavioral health disorders, human growth and development, evaluations, assessments, treatment planning, basic counseling and behavior management interventions, case management, clinical record documentation, psychopharmacology, abuse regulations, and recipient rights.
  • Work under the supervision of a master’s level practitioner.

  • Must have reliable transportation, valid driver's license and required automobile insurance.
  • Must have active cell phone at all times.
  • Must meet standards of Level I and Level II criminal background screening in accordance with FL Statutes (F.S.) 435.04 and (F.S.) 408.809.

Provides direct patient care to clients with behavioral or emotional disorders under supervision of a licensed clinical professional and established protocols of FTS. Perform basic patient education and data gathering activities.

Essential Functions

  • Work as part of a health care team to assess patient needs, plan and modify care and implement interventions.
  • Evaluate nursing intervention outcomes, conferring with other health care team members as necessary.
  • Perform nursing assessments on all psychiatric and medication management appointments.
  • Obtain and record patients’ weight, blood pressure, medical history and assess reason for patient visits.
  • Record and report observed symptoms, reactions, treatments, and changes in the patient’s condition.
  • Administer medication injections as needed.
  • Answer incoming calls from patients regarding medication questions and concerns.

Scope of Position: Diagnoses, prescribes medication, monitors medication side effects and participates in the treatment team process.

Essential Functions

  • Completes Psychiatric Evaluations.
  • Prescribe and direct medications to treat mental, emotional, or behavioral disorders.
  • Examine and review diagnostic test on patients to provide information on general physical condition or mental disorders.
  • Performs clinical screening and referral of all conditions in need of consultation from medical, non-psychiatric specialties.
